What the questionnaire collects
When you submit the questionnaire, Lifestyle Dentistry receives:
- Your answers to the 12 questions about how you breathe, how you sleep and how you feel during the day. Every question is optional.
- Today's date, as entered on the form.
- Your first and last name, if you give them.
- Your email address.
The form also records when it was submitted, the page it was submitted from and your browser type. It does not store your IP address.
How it is used
Dr. Aja Nichols and the Lifestyle Dentistry team review your responses and follow up with you about what they may suggest and whether a closer look at your sleep and airway could make sense. Your answers aren't a diagnosis.
By submitting the questionnaire, you consent to receive marketing emails from Lifestyle Dentistry. You can revoke your consent at any time by using the unsubscribe link at the bottom of every email.
Spam protection
To keep automated spam out, submissions are checked by CleanTalk and Google reCAPTCHA. Both receive the details they need to check a submission, such as your email address and IP address, and may use cookies. Google reCAPTCHA is subject to the Google Privacy Policy and Terms of Service.
